Ray presented to the office in September 2025 complaining of significant chronic and worsening pain in his lower back and down the left lateral hip/thigh and often to his leg. It had started approximately 6 months before going to our office and he had “failed out of” Physical Therapy . Ray is retired, plays golf several times a week with friends, and exercises 4-5 days per week at the park district health club. Because of his problems, he had been unable to play more than 9 holes of golf in one setting without taking a cart, was unable to jog and/or walk the track at the health club, and was unmotivated to exercise with weights/resistance training in the fitness center because he felt worse all of the time with these activities.
