    Results Assurance™ Terms & Conditions

    Please review these terms carefully before activating Results Assurance™. By activating, you agree to be bound by these terms.

    Important

    Results Assurance™ provides a platform-issued care credit and is not a cash refund, insurance product, or guarantee of clinical outcomes. Credits are issued at ChiropracticResults' sole discretion.

    Program Overview

    • Results Assurance™ is a patient confidence program offered by ChiropracticResults. It provides a platform-issued care credit — not a cash refund, insurance product, or guarantee of clinical outcomes.
    • If a patient's progress stalls after completing a provider-defined care milestone and demonstrating reasonable compliance with care instructions, the patient may qualify for a care credit of up to $500 to continue care with another verified provider on the ChiropracticResults platform.
    • Results Assurance™ is designed to reduce booking hesitation by giving patients a safety net when choosing a new provider.
    • All adjudication and credit issuance decisions are made solely by ChiropracticResults.

    Eligibility

    • The patient must book their initial appointment through ChiropracticResults or a ChiropracticResults-affiliated booking flow.
    • Results Assurance™ must be activated before care begins. Retroactive activation is not available.
    • The provider must hold an active HYPE-tier subscription on the ChiropracticResults platform at the time of activation.
    • Only one Results Assurance™ activation is permitted per patient per provider.
    • The patient must provide accurate contact information (name, email, phone) during activation.

    Qualification Criteria

    • To qualify for a care credit, the patient must meet all of the following conditions:
    • Complete the provider-defined care milestone (e.g., a specific number of visits or treatment duration as outlined in the care plan).
    • Demonstrate reasonable compliance with the provider's care instructions, including attending scheduled appointments and following at-home recommendations.
    • Not achieve the desired clinical results as determined by a good-faith review by ChiropracticResults.
    • Submit a claim within 30 days of completing the care milestone.
    • ChiropracticResults may request documentation from the provider or patient to verify compliance and outcomes.

    Credit Details

    • The care credit is valued at up to $500 and is issued at ChiropracticResults' sole discretion based on the circumstances of the claim.
    • Credits may only be applied toward care with another verified provider listed on the ChiropracticResults platform.
    • Credits are not redeemable for cash, are non-transferable, and have no cash equivalent.
    • Credits must be used within 90 days of issuance. Expired credits cannot be reinstated.
    • Credits cannot be combined with other promotional offers unless explicitly stated.

    Limitations

    • Limit one care credit per patient across all providers on the platform.
    • Results Assurance™ is not a guarantee of clinical outcomes. Individual results vary based on age, condition severity, overall health, and adherence to treatment.
    • ChiropracticResults reserves the right to approve, deny, or adjust any claim at its sole discretion.
    • The program may be modified, suspended, or discontinued at any time without prior notice.
    • Results Assurance™ is not an insurance product and is not regulated as such.

    Claims Process

    • To submit a claim, contact ChiropracticResults at twisty@thespizz.com or text 323-627-2612 within 30 days of completing your care milestone.
    • Include your full name, email used during activation, provider name, and a brief description of the outcome.
    • ChiropracticResults will review the claim within 14 business days and may contact the provider and/or patient for additional information.
    • If approved, the care credit will be issued and the patient will be notified with instructions on how to apply it toward a new provider.
    • All decisions are final and are not subject to appeal.

    Exclusions

    • Claims will not be honored in any of the following circumstances:
    • The patient did not complete the provider-defined care milestone or failed to attend scheduled appointments.
    • The patient did not follow reasonable care instructions provided by the treating provider.
    • The patient did not activate Results Assurance™ before care began.
    • The patient provided false or misleading information during activation or the claims process.
    • Pre-existing conditions were not disclosed to the treating provider at the start of care.
    • The patient seeks a cash refund rather than a care credit.
    • The claim is submitted more than 30 days after completing the care milestone.
