    "Sciatica" Isn't a Diagnosis — It's a Symptom

    When someone walks into Best Life Chiropractic in San Diego saying "I have sciatica," we already know one thing for certain: they have leg pain, numbness, or tingling that follows the path of the sciatic nerve. What we don't know yet is why — and that "why" is what determines whether relief takes 3 weeks or 3 months.

    The Three Most Common Root Causes We See

    1. Lumbar Disc Involvement (L4-L5 or L5-S1). A bulging or herniated disc presses on the nerve root as it exits the spine. Patients often describe pain that shoots below the knee and is worse with sitting, sneezing, or bending forward. This is the classic "10-year sciatica" story we see resolve once decompression and specific adjustments unload the disc.

    2. Piriformis Syndrome. The sciatic nerve passes through (or under) the piriformis muscle deep in the glute. When that muscle stays tight — usually from prolonged sitting at desks in Sorrento Valley or long commutes on the 5 — it compresses the nerve. Pain typically stays in the buttock and back of the thigh, and rarely goes past the knee.

    3. Sacroiliac (SI) Joint Dysfunction. The SI joint is often the missed culprit. Pain sits at the dimple of the low back and refers into the groin or upper thigh. Postpartum patients and runners are the most common presentations at our office.

    Why the Diagnosis Changes Everything

    A herniated disc treated as "tight glutes" will not improve. A piriformis-driven case treated with disc decompression won't either. In our exam we combine orthopedic testing (SLR, Slump, FABER, Gaenslen's), neurological screening (reflexes, dermatomes, myotomes), and — when warranted — imaging referrals before we ever adjust.

    What Effective Sciatica Care Looks Like

    • Reduce nerve pressure first. Flexion-distraction or spinal decompression for disc cases; targeted soft-tissue work for piriformis cases.
    • Restore joint motion. Specific, low-force adjustments to the lumbar spine, SI joint, and hips.
    • Rebuild. Glute activation, hip mobility, and core endurance so the pain doesn't come back the next time you sit through a long meeting.
